If you delayed your Part B and are now signing up, make sure to add the Part B start date that you would like in the REMARKS section of the application. If you are losing employer coverage, you will want to select the 1st of the following month after losing coverage. For example: If you are losing your employer coverage on September 30, you would want to select October 1 as your start date. You will also need to get this form completed by your employer. CMS L564
There are two ways to apply for delayed Part B. Online with document uploading or print then hand deliver to your nearest Social Security office. (You can actually fax it too if you trust that method.) If you're tech savvy and can upload documents, online may work for you. If you are not, print the form, gather the documents, and make an appointment with Social Security. Feel free to reach out if you need assistance.

*Elder Affairs- This site is for help with Medicaid LTC. There is a waiting list so apply sooner than later if you are in need of LTC assistance. They also assist with energy assistance and transportation, among other things—upon qualification.
